Pt. 1 Dx. F/79 CVA with В® hem paresis on dominate side.

LTG. Pt. to safely perform self care tasks and functional mobility

with SBA.

STG. 1. Pt. to improve (B) UE AROM and functional strength demonstrated by В® shoulder abd. 50%, В® shoulder flexion

45 degrees, В® elbow flexion 105 degrees, В® elbow extension

-42 degrees.

2. Pt. to safely transfer to all functional surfaces with SBA.

3. Pt. to improve standing bal. to F+

4. Pt. to perform UB ADLs with min A. (may require assistive

technology)

Activities Various Thera band exercises to increase UE strength. This will

increase functional strength, flexion, extension, and also aid

pt. with safely transferring.

Cones to be passed behind head from one hand to the other. This will aid shoulder flexion, extension, and ROM to help perform

UB ADLs.

Standing at sink. This will improve standing tolerance, and bal. to

aid in goal of standing bal., transfers, and to perform ADLs.

Teach safety techniques for transferring. This will aid in

performing safe transfers.

Teach proper technique for using adaptive equipment. This will

aid in successful use of adaptive aids.

Pt. 2 Dx. F/67 Osteoarthritis and overall muscle weakness.

LTG. Increase Pt. strength, bal., and ADLs to reach max. level

of func. (I).

STG. 1. Pt. to increase UE strength to 4/5.

2. Pt. to increase UB ADL functioning to 5.

3. Pt. to transfer safely to all functional surfaces with SBA.

4. Pt. to increase functional activity tol. to 15 min. with 1 RB.

Activities Various exercises with 1lb. weight to increase strength. This

will help increase UB functioning, help to increase safety in transfers, ADL functioning, and improve overall muscle

strength in UB.

Hand bike. This will increase tol., UE flexion and extension,

and help improve ADL functioning.

Standing at sink. This will improve standing tolerance, and bal. to

aid in goal of standing bal., transfers, and to perform ADLs.

Teach safety techniques for transferring. This will aid in

performing safe transfers.

Pegboard. This will help fine motor movement in hands to assist

in ADL functioning.

ROM arc. To improve ROM, to assist in ADL functioning.

Pt. 3 Dx. F/91 Acrocallosal Syndrome (ACS) and overall muscle weakness.

LTG. Pt. will safely perform ADLs, IADLs, and functional mob. with

modified (I).

STG. 1. Pt will safely transfer to all functional surfaces with mod (I).

2. Pt. to improve standing bal. to F+, static/dynamic.

3. Pt to improve functional activity tol. demonstrated by standing

10 min. while participating in therapeutic activity.

4. Pt to perform toileting taks with mod (I).

5. Pt. will safely perform IADLs with SBA.

Activities Standing at sink. This will improve standing tolerance, and bal. to

aid in goal of standing bal., transfers, and to perform ADLs.

Teach safety techniques for transferring. This will aid in

performing safe transfers.

Repetition of activities. This will aid someone with ACS in successful learning of new techniques for transferring and safety.

Toilet transfers. This will help in gaining independence for toileting.

Simulate washing dishes. This will aid in standing bal. combined with a functional activity.

Thera band exercise. This will increase UB strength to aid in safe transfers.

Pt. 4 Dx. F/87 COPD, and overall muscle weakness.

LTG. Increase Pt. strength, tol, bal. to reach max functional (I) of ADLs.

STG. 1. Pt. to increase activity tol. to 15 min. with 1 rest break.

2. Pt. will perform UB ADLs with min A.

3. Pt to increase (B) shoulder flex. strength to 3+.

4. Pt. to increase LB ADLs to mod A.

5. Pt. to perform functional transfers with good safety.

Activities Hand gripper. To increase strength in hands to help with safe transfers, and ADLs.

ROM arc. To increase (B) shoulder ROM, and to increase activity tolerance.

Sit to stand. This will aid in activity tol., and safely performing transfers.

Thera band exercise. This will increase UB strength to aid in safe transfers, (B) shoulder flexion, and activity tol.

Dressing activity. This will aid in LB ADLs and also

help to teach Pt. how to conserve energy.

Breathing activity. This will teach Pt. proper technique to

breath due to COPD.

Safety education. This will help teach proper rest breaks and also transfer techniques due to COPD.

Pt. 5 Dx. F/77 Femur fracture (L)

LTG. Pt. to perform all ADLs with mod (I), demonstrate safe functional

mobility with mod (I) (upon clearance for WB status).

STG. 1. Pt. will improve sitting bal. to "good" both static and dynamic.

2. Pt. will safely perform UB ADLs with SBA.

3. Pt. will improve bed mobility demonstrated by supine to sit/

sit to supine with SBA.

4. Pt. will safely demonstrate sit to stand with SBA with AE in compliance with NWB status for (L) LE.

Activities Reaching for cones. This will improve sitting bal. static/dynamic,

flexion and extension of UE to improve UB ADLs, and bed

mobility.

Thera band. This will increase UB strength to aid in sit to stand,

bed mobility, and UB ADLs.

Safety education. This is to educate Pt. on WB precautions, and transfer precautions.

Sit to stand. This improves balance, activity tol. UB strength, to aid in sit to stand, bed mobility and UB ADLs.

Sitting reclines. This is to strengthen trunk to improve balance and bed mobility.

Pt. 6 Dx. M/81 CHF, and overall muscle weakness.

LTG. Pt. will increase strength, tolerance, balance, and achieve max level of ADL functioning.

STG. 1. Increase (B) UE strength to 4/5

2. Increase UB ADLs to mod A.

3. Pt. to increase activity tolerance to 15 min. of functional

activity with 1 RB.

4. Pt to increase functional transfer to mod Ax2 with good safety.

Activities Weight bar 2lbs. To improve UB strength, aid in UB ADLs, to increase activity tolerance, and aid in functional transfers.

Feeding activity. To increase ADLs, and activity tolerance.

ROM arc. To increase (B) shoulder ROM, and to increase activity tolerance.

Sliding board. Aids in functional transfers, UB strengthening, and increases activity tolerance.

Bed Mobility. To increase functional transfers, activity tolerance,

and improve UE rom and strength.

Pt. 7 Dx. F/79 Hip Fracture, and muscle weakness

LTG. Pt. to safely perform self care tasks as well as functional mobility with SBA or less, in compliance with hip precautions as long as appropriate.

STG. 1. Pt. will improve bed mobility (I), demo by rolling with min A, supine to sit, sit to supine with min A.

2. Pt. to improve sitting bal. to F+ static/dynamic.

3. Pt. will safely transfer to all functional surfaces with mod A.

4. Pt. will safely perform UB ADLs with SBA.

5. Pt. will self feed with SBA.

Activities Sitting reclines. This is to strengthen trunk to improve balance and bed mobility.

Feeding activity. To increase ADLs, and activity tolerance.

Thera band. This will increase UB strength to aid in sit to stand,

bed mobility, and UB ADLs.

Reaching for cones. This will improve sitting bal. static/dynamic,

flexion and extension of UE to improve UB ADLs, and bed

mobility.

Safety education. This is to educate Pt. on WB precautions, and transfer precautions.

Bed Mobility. To increase functional transfers, activity tolerance,

and improve UE rom and strength.

Pt. 8 Dx. F/51 Osteoarthritis and overall weakness

LTG. Pt. will increase strength, tolerance, ROM, and ADLs to reach max level of functioning (I).

STG. 1. Pt. to increase grooming ADL to mod A.

2. Pt. to tolerate В® hand splint x 1hr. 3-5 x week.

3. Increase В® hand digit extension to -30 degrees.

4. Pt. to increase activity tolerance to F+.

5. Pt. to increase functional activity to 15 min. with 1 RB.

Activities PROM. This is to maintain mobility in В® elbow, wrist, fingers.

Stretching fingers and wrist. This increases extension in joints being stretched.

Splinting. To prevent Pt's В® hand from contracting.

Pt. 9 Dx. M/78 CVA and muscle weakness.

LTG. Pt. will safely perform self care and functional mobility with mod (I) and ADLs with mod (I).

STG. 1. Pt. will safely transfer to all functional surfaces with SBA.

2. Pt. will improve sitting balance to F+ static/dynamic.

3. Pt. to improve bed mobility demo. by supine to sit, sit to supine with mod (I).

4. Pt. will perform basic UB ADLs with SBA.

5. Pt. to improve standing balance to F+ static/dynamic.

Activities Standing at sink simulate shaving. This will improve standing tolerance, and bal. to aid in goal of standing bal., both static/dynamic, transfers, and to perform ADLs.

Bed Mobility. To increase functional transfers, activity tolerance,

and improve UE rom and strength.

Reaching for cones. This will improve sitting bal. static/dynamic,

flexion and extension of UE to improve UB ADLs, and bed

mobility.

Hand bike. This will increase tol., UE flexion and extension,

and help improve ADL functioning.

Various Thera band exercises to increase UE strength. This will

increase functional strength, flexion, extension, and also aid

pt. with safely transferring.

Teach safety techniques for transferring. This will aid in

performing safe transfers.

Pt. 10 Dx. F/89 CHF and overall weakness.

LTG. Pt. will reach max. level of functioning (I) in ADLs.

STG. 1. Pt. to improve UB ADLs to min A.

2. Pt. to increase LB ADLs to mod A.

3. Pt. to increase (B) shoulder flexion to 3+/5.

4. Pt. to increase activity tolerance to 15 min of functional activity with 1RB.

5. Pt. to increase functional transfers to mod A with good safety.

2. Pt. to increase (B) UE functional strength to 4/5.

3. Pt. will safely transfer from bed to wc, wc to bed with min A with sliding board.

4. Pt. will improve bed mobility rolling with SBA, supine to sit, sit to supine with min A.

Activities Sitting reclines. This is to strengthen trunk to improve balance and bed mobility.

Thera band. This will increase UB strength to aid in transfers,

bed mobility, and UB ADLs.
