Using the Nursing Process in Everyday Problem-Solving
I. Assessment
A. Objective Data
1) Patient is limping and grimaces, as if in pain, each time he bares weight on left leg.
B. Subjective
1) Patient states that his leg hurts. Each individual's perception of pain varies.
II. Analysis
A. Pain in left leg after bike accident, in which protective gear not used.
III. Plan
A. Short-Term Goal: Patient will be examined for possible sprain or stress fracture
B. Long-Term Goal: Patient will rest knee for at least 24 hours, or until pain and swelling
desist. After 24 hours, allow any activity that does not cause pain.
IV. Implementation
A. Apply a cold Pack to afflicted area to relieve pain and to help reduce swelling.
(To avoid direct contact with the skin, wrap the cold pack in a towel)
B. Keep the injured leg elevated above the level of the heart. Use a pillow to help
elevate the injured leg.
C. Encourage patient to reduce or stop using the injured leg for at least 24 hours. When
patient able to resume similar activity, all necessary protective gear should be worn.
V. Evaluation
A. Short-Term Goal: Achieved through examination of left leg, no damage to ligaments or
cartilage.
B. Long-Term Goal: None necessary. Advised parents to limit patient's activity until pain
and swelling cease.
