This release is made on the ____ day of __________, 2004 by

________________________________________ (hereinafter "Guest") in favor

of _______________________ (hereinafter "Producer").

For good and valuable consideration, the receipt and sufficiency of which are hereby acknowledged, Guest grants Producer the right to make both visual shadow and/or audio recordings of him/her which are not recognizable (collectively, the "Photography") on the above date and any other times mutually agreed upon and the perpetual, worldwide right to use the Photography, in whole or in part, and his/her un-recognizable voice, shadow, and biographical information in connection with Producer's production presently entitled "___________________________", and in the advertising and promotion thereof (collectively referred to as the "Production").

This license shall not include the right to use the Photography independently of the Production.

Guest agrees that the rights granted hereunder shall include the perpetual, worldwide right of Producer to edit, telecast, cablecast, rerun, record, publish, reproduce, use, license, print, distribute or otherwise exploit, in any manner and in any medium or forum whether now known or hereafter devised said Production, in whole or in part, and any versions thereof.
